One
of the main sources of information is a version of a chapter I've
written for a text book on Neuro-Ophthalmology. I have attempted
to keep this updated as it takes two years to get a chapter
published. Therefore this version is not what appears in print. It
is meant primarily for physicians although I've given earlier
versions to many of my patients. Illustrations have now been
scanned and appear with the text, ditto for tables. The numbers in
brackets {} in the text refer to citations which come at the end.
